Can I be treated the same day?

Usually yes. The on-site test gives an answer within minutes, and where symptoms and that result agree, treatment starts immediately rather than waiting for the culture. The culture then confirms the choice was right.

Why send a culture if I am already being treated?

Because antibiotic resistance is common enough that the first choice sometimes fails. The culture identifies the organism and which antibiotics it responds to, so if you are not better in 48 hours the next step is already known rather than guessed.

When is this urgent rather than routine?

Fever, pain in the back or flank, vomiting, or feeling genuinely unwell suggest the infection has reached a kidney, and that is not a wait-and-see situation. Say so when you book rather than at the appointment.

I keep getting these. Is there anything else?

Yes, and repeated courses of antibiotics are not the answer on their own. Three or more in a year is a reason to ask why it keeps happening, which is a different conversation and sometimes a different investigation.

How it works

The culture is why treatment works.

A dipstick on a urine sample gives an answer in two minutes: it detects nitrites, which most urinary bacteria produce, and white cells, which mean inflammation. That is enough to start treatment the same day when symptoms are clear.

What it cannot do is say which organism is responsible or which antibiotic will kill it. A culture does: the sample is grown, the organism identified, and it is tested against a panel of antibiotics. That matters more each year, because resistance is rising and the antibiotic that worked for everyone a decade ago now fails in a meaningful share of cases. Treatment often starts on the dipstick and is adjusted when the culture comes back — which is why you are contacted even when you are already feeling better.
